Health Insurance-UMR

Miami offers two health plan options through UnitedHealthcare, UMR. You choose the plan that is best for you and your family. Option 1 is the Traditional PPO and option 2, the High Deductible Health Plan (HDHP) with a Health Savings Account. Both plans include pharmacy coverage.

UMR Member Card

To order a new card, log in to your UMR account and do the following:

  1. Under “myMenu,” select ID card.
  2. Next, select Order ID card.

Monthly Premiums

Use the 2019 Premium Calculator to calculate your own premium.

Single

PPO, 1.32% x Annual Salary divided by 12 + $63.00 = Monthly premium
HDHP, 1.32% x Annual Salary divided by 12 = Monthly premium

Employee Plus Spouse or Children

PPO, 2.90% x Annual Salary divided by 12 + $139.00 = Monthly premium
HDHP, 2.90% x Annual Salary divided by 12 = Monthly premium

Family

PPO, 4.22% x Annual Salary divided by 12 + $203.00 = Monthly premium
HDHP, 4.22% x Annual Salary divided by 12 = Monthly premium

Please note: Premiums will be no more than 30% of the total monthly premium.

An additional amount applies to your health premiums if you and/or your covered spouse chose not to complete the steps of the premium discount program in the previous calendar year.

Depending on the steps not completed, your premiums could include an additional $15-$60 premium per month for you and an additional $15-$60 premium per month for your spouse.

Who Can Be Covered?

Changing Your Coverage

You can change your health coverage during the benefit open enrollment period in October for the following calendar year, or within 31 days of a qualifying event with appropriate documentation.
