Background: In urinary and renal hydronephrosis, if
the prolonged congestion was not resolved, it will
lead to irreversible damage
and glomerular filtration rate could decline gradually
over time to end-stage
renal disease. We conducted this study with
the aim: evaluate the renal function in the hydronephrosis patients by scintigraphy
or glomerular filtration rate calculated by Cockcroft-Gault and MDRD.

Patients and method: The prospective study
was performed at
the Neph-Urology
Department, Bach Mai Hospital from 9/2011
to 8/2012 on 44
patients who were
diagnosed urinary and renal hydronephrosis.

Results: By renal scintigraphy, total GFR of 2 kidneys showed with
65.9% lower than 60 ml/min/1.73m2. In the damage kidney, there were 40/41 cases with glomerular filtration rate decreased and the
mean value was 11.6 ± 11.2 ml/min/1.73m2.
MDRD formula results:
26 of 44 patients
(59.1%) having eGFR<60 ml/min/1.73m2 with the mean value
was 15.8 ± 17.3 ml/min/1.73m2.
The results of Cockcroft-Gault formula: 36/44 patients
(81.8%) with eGFR<60 ml/min/1.73m2, the mean value was 23.3±21.2 ml/min/1.73m2. In the comparison the results of eGFR identified
by three methods: The patients
<60 ml/min/1.73m2
calculated by Cockcroft-Gault formula was highest number
compared to the results of MDRD formula
and scintigraphy. There were
similarities when determining eGFR by MDRD formula
and the results
of renal scintigraphy to determine the
proportion of patients with eGFR
<60 ml/min/1.73m2.

Conclusions: In our
study, eGFR was
decreased in all patients’ urinary and renal hydronephrosis. Renal
scintigraphy allowed assessing separately GFR in each kidney and the total GFR of both two kidneys, so in general
that helped for having accurate treatment. MDRD
formula estimated the GFR relatively close
to the GFR as determined by renal scintigraphy.
